Community Health Partners provides outpatient physical and aquatic therapy services and we are dedicated to improving our patients’ quality of life. All our therapy programs are designed with personalized wellness goals in mind. Not only do we strive to return each patient safely and quickly to their previous level of function, we also provide medically-guided exercise and wellness programs for patients after therapy to address long-term healthy living needs. Our physical therapists excel in the following areas:

Injury Screens – this is a one-time consultation with a physical therapist to assess a specific injury or problem. This meeting is free of charge and not billed to insurance.
Land Based and Aquatic Based Physical Therapy – this is a full medical service provided by a physical therapist and billed through insurance. This includes an evaluation and resulting therapy treatment sessions based on the impairment found. After the initial evaluation, the physical therapist can determine if the individual is appropriate for aquatic and/or land based treatments. Treatments can be focused on regaining strength, range of motion, balance, and pain management. Independent exercise programs will be developed so that patients who graduate from physical or aquatic therapy will be able to continue working on their exercise plan independent of a physical therapist. Payment is through insurance; however, patients could be responsible for a copay depending on their insurance plan. Cash-based options are also available.
Annual PT Check-Ups – meet with a physical therapist to identify and address issues before they progress into joint or muscle pain. One of our CHP Physical Therapists can assess your current physical status and identify any early signs of impairment or risk for disease. Leave with a plan to help prevent an injury, decrease your risk for disease or enhance your current workout regimen.
Population Health Physical Therapy – geared towards high risk patient populations who need assistance getting started with independent exercise. A brief stint of physical therapy specifically targeted at helping an individual exercise safely with possible adaptations or modifications to equipment, environment, class participation, monitoring exercise tolerance, and assistive device/support. This medical service includes an evaluation with a physical therapist to design an independent land/water exercise program with subsequent visits scheduled to help the patient learn their new program and be sure they are safe and capable to continue on their own.
